Failure Modes and Effects Analysis (FMEA)

FMEA shows you failure risks before you start. With a little brainstorming the effects and causes of failure are determined along with the appropriate controls. If you are building a fighter pilot ejection system, wouldn’t you like to know this in advance?

Above all the FMEA takes into consideration failure impact, failure severity, failure causes, mitigation controls, and ability to detect.

At the end of the exercise you will have a risk mitigation strategy based on the impact of the greatest potential failure.
